Singaporean startup Castomize has developed a 4D-printed orthopedic cast that uses smart thermoplastics to mold directly to a patient's anatomy at the point of care, eliminating the need for prior 3D scanning or a saw for removal. The device, originating from research at the Singapore University of Technology and Design, became a commercial product in 2022 and is now sold for wrist, forearm, arm, and ankle applications. The cast is printed in standard sizes with a fixed geometry, then softened with heat by the clinician, molded over the limb, and hardened upon cooling. Its open lattice structure provides breathability, waterproofing, and adjustability — if swelling changes, the clinician can reheat and remold the same piece without replacement. Castomize is expanding its catalog into prosthetics and veterinary medical devices, including pediatric sizes.
This development represents a rare commercial translation of 4D printing — structures that change shape under external stimuli — into a viable medical product. The cast addresses the core drawbacks of conventional fiberglass casts: itching, sweating, inability to get wet, and difficult removal. By removing the need for patient-specific scanning and enabling in-clinic adjustment, Castomize lowers the barrier to adoption for clinics and healthcare professionals. The product sits at the intersection of polymer vat photopolymerization and material extrusion, leveraging smart thermoplastics rather than rigid photopolymers or powder-based processes. While 4D printing has been explored in research settings for years, Castomize's approach — standard sizes with post-print customization — avoids the cost and complexity of full patient-specific scanning, making it more practical for high-volume clinical workflows. The company's move into veterinary and pediatric applications signals an attempt to broaden the addressable market beyond adult orthopedic care.
From an AM industry perspective, Castomize's cast is a pragmatic application of 4D printing principles rather than a technological breakthrough. The key execution challenge is scaling production of the smart thermoplastic lattice structures while maintaining consistent material properties across batches. For clinicians, the value proposition is clear: reduced patient discomfort, fewer follow-up visits for cast replacement, and simpler removal. The company must now demonstrate reliability across a larger patient population and secure reimbursement pathways to drive adoption beyond early-adopter clinics.
